This application should be used by hospitals and nursing homes that are in the potential evacuation area along the South Carolina coast to file information about their emergency preparedness.

In the event of a category 3 or less hurricane this application will help DHEC determine whether the facility needs to be evacuated. Without a critical data sheet application on file, the hospitals and nursing homes must evacuate.
