This application should be used by hospitals and nursing homes that are in eight coastal counties in the state of South Carolina to file information about their emergency preparedness for hurricane evacuation. The eight counties are Horry, Georgetown, Charleston, Berkeley, Dorchester, Colleton, Beaufort, and Jasper.

In the event of a category 3 or less hurricane this application will help DHEC determine whether or not a facility needs to be evacuated. Without a critical data sheet application on file, the hospitals and nursing homes must evacuate.
