SC DHEC - Critiacal Data Information

This application is to be used by the facilities listed below to file information about their emergency preparedness for hurricane evacuation and other disaster/emergency events that may involve partial or full facility evacuation:

  1. Hospitals in the state of South Carolina; and
  2. Nursing homes that are in eight coastal counties in the state of South Carolina (the eight counties are Horry, Georgetown, Charleston, Berkeley, Dorchester, Colleton, Beaufort, and Jasper).

In the event of a hurricane threat, this application will help DHEC and the South Carolina Emergency Management Division (SCEMD) determine whether or not a facility needs to be evacuated. Without a critical data sheet application on file, the hospitals and nursing homes must evacuate.
