In an effort to increase awareness of the prevention and management of diabetes, The Diabetes Initiative of South Carolina (DSC) formed a partnership with South Carolina Diabetes Prevention and Control Program (SC DPCP) to form local diabetes coalitions. These coalitions were formed throughout the four South Carolina Department of Health and Environment Control (SC DHEC) created geographic regions (Coastal, Pee Dee, Midlands, and Upstate) in the fall of 1999. After establishing the local coalitions, DSC and SC DPCP identified the needs for linkage between the coalitions throughout the state. Description of Initiative:
The EXPORT Center on Metabolic Syndrome and Minority Health is a partnership for excellence in outreach, research and training between the Medical University of South Carolina (MUSC) and South Carolina State University (SCSU). Theme: The partnership focuses on the metabolic syndrome, a cluster of disorders, including hypertension, diabetes, lipid abnormalities and obesity, that have a strong impact on health disparities. Objectives: The partnership addresses the principal objectives of interest to the National Center for Minority Health and Health Disparities (NCMHD). Specific Aims: 1. Promoting the conduct of research to reduce health disparities – The metabolic syndrome is a major contributor to health disparities and has its origin in unhealthy lifestyles established early in life. Our Center focuses on the metabolic syndrome and healthy lifestyles in youth and young adults. 2. Building research capacity for health disparities research in minority-serving institutions – Our Center proposes two distinct, inter-related areas of multi-disciplinary, inter-institutional collaboration. a. Community-based activities that integrate extramurally funded programs at MUSC and SCSU, using empowerment model and community-based participatory research approach to leverage existing efforts to address health disparities at the grass-roots level (Outreach Core). b. Multi-disciplinary translational research on lifestyle change and the metabolic syndrome as well as the impact of behavioral early developmental (birth weight), and genetic factors on the expression of the syndrome and responses to lifestyle change (Research Core). This activity will leverage efforts and build capacity through: i) Developing a new Clinical Research Unit at SCSU (Shared Facilities Core). ii) Capitalizing on up to seventy minority research supplements available for career development of faculty and students at SCSU and MUSC (Training Core and Pilot Research Core). 3. Promoting participation of health disparities groups in biomedical and behavioral research prevention and intervention activities – the teaching programs about the metabolic syndrome and healthy lifestyles (Education Core), as well as the community-based activities in the Outreach Core and translational research activities in the Research Core, all center on African American youth and young adults in the community and/or students at SCSU. 4. Fostering broad, inter-institutional, collaborative, multidisciplinary investigation that increases Project EXPORT’s effectiveness and impact – the talent, commitment, and leadership of the two institutions, coordinated by the Administrative Core, are in place to realize the tremendous potential for reducing health disparities through an EXPORT Center led by MUSC and SCSU.

The South Carolina AIDS Drug Assistance program (SC ADAP) was established under the Ryan White CARE Act to provide drugs to treat HIV disease and/or to prevent the serious deterioration of health arising from HIV disease in eligible individuals, including measures for the prevention and treatment of opportunistic infections and document the progress made in making the drugs available. The SC ADAP is operated through a centralized pharmacy and an insurance assistance program located at the Department of Health and Environmental Control. Currently 57 drugs are on the approved formulary.

The Hypertension Initiative is a quality-of-care improvement program using an electronic database to help primary care providers track treatment and outcomes in their patients at high cardiovascular risk. Participation is free of charge and open to any practice regardless of whether electronic or paper charts are being used. Participating physicians receive regular feedback reports, a quarterly newsletter, and access to the Transworld Electronic Medical Record system (TEMR) which is a low/no-cost electronic medical record, billing, and scheduling system that was developed to help clinics without an EMR system to participate particularly community health centers and small rural practices.

Description of Initiative:
An over arching goal of this program project grant (PPG) is to move beyond the identification and description of African American/Caucasian health-related disparities to understanding the disparities within the African American population and identifying solutions designed to address these health issues. The theme is to improve the process of health care delivery for African Americans by addressing issues and interventions related to providers and provider/patient interactions. PPG investigators are testing new interventions and, if the interventions are successful, will move the interventions forward to statewide implementation. Knowledge in South Carolina will contribute to the reduction of health disparities for African Americans in the nation. The unique state database linking patient and provider information provides data essential to the PPG goals. The PPG is also engaging new minority and non-minority investigators in minority health research. Four Major Projects: 1. Reducing the Racial Disparity in Cardiovascular Disease Through Better BP control. 2. Economic Analysis of Access To Anti-retroviral (ARV) Prescription Drugs & Adherence To ARV Guidelines For African American Medicaid Enrollees With AIDs or HIV-Disease In South Carolina. 3. An Exploration of Racial Differences in End-of-Life Care Preferences Among Cancer & Congestive Heart Failure Patients in SC 4. A Randomized Trial of a Pharmacist Intervention to Increase Prescription Refills.
