Examples Of The Minority Health Disparity Initiatives Database

How to Add an Initiative

To add an initiative to the database click on the add link at the top of the browser. This will send you to the add page which has the form to be filled out. As seen, in part, in the image below.

Blank Example

To fill out the form, please enter the information specific to each question in the spaces provided. Questions marked with a red astric are required to be filled out. Please do not include any apostrophes or single quotes in the responses. If you do not know the required information please type in DK, meaning don't know. An example of a filled in section is shown below.

The form also contains check boxes, radio buttons, and drop down lists. At least one item must be selected to submit the form. If the answer to the question is not listed, check the other box and fill in the appropriate answer. An example of the check box, radio button, and list box is shown below.

Checkbox Example

Once all information has been filled in, click submit on the bottom of the page. A warning may pop-up reminding you to fill in any forgotten fields. Once all required fields have been checked or filled in, you will be directed to the preview page. In this page you have the opportunity to preview the data and ensure the accuracy of what will be submitted to the database. If any information is incorrect or mis-typed then the user can click the back button on the browser window and fix the error. Once this has been done and re-submitted, you can then submit the initiative into the database. A thank you screen appears and you can then add another initiative, query the database, read the supporting page information, or leave the site. The initiative will not be posted publicly until it has been review by the Office of Minority Health. This ensures correct entries and proper use of the web site. Once reviewed and posted, the initiative will be publicly seen. If there is information that is not wanted to be shown publicly, please contact the Office of Minority Health and they can remove this field or information.


How to Query the Database

Similar to the add page, the query page has a series of check boxes, radio buttons, and drop down lists each of which further restrict the query. If you wanted to look at initiatives pertaining to Adults ages 26-46 within Richland County, You would simply check Adults 26-46 in the age group selection area and select Richland County in the drop down list. After which you would click submit at the bottom of the browser window. An example of this the county selection box is is shown below.

Filled Example

The results of the query would be returned along with a list of what was queried at the top of the page (shown below). If the resulting query posted no results, or if a wider search range is wanted, simply return to the query page and re-select. If all initiatives are wanted please scroll down and click the submit button in the query page without changing any of the defaults settings. This will return all posted initiatives entered into the database.
